UTAH FIREARMS INSTRUCTOR APPLICATION PROCEDURES

How to Apply

Instructor applicants must complete all applicable sections of Bureau form 96-4-02 - Application for Concealed Firearm Permit. (Requires Adobe Reader. To download the FREE Adobe Reader, visit the Adobe® web site.)

Cost to Apply

A non-refundable fee of $5.00 must be included with your application. Please make checks and money orders payable to the "Utah Bureau of Criminal Identification".

What Must Accompany the Firearms Instructor Application?

  • Attach a copy of any firearms certification you currently hold, which may include the N.R.A. Firearms Instructor Certificate, P.O.S.T. Firearms Instructor Certificate, or equivalent insurer certification. If you are not certified through N.R.A. or P.O.S.T., but you have equivalent training and certification, submit a copy of the course outline that qualifies you to be an instructor.
  • Photo copy of Drivers License or State ID
  • Attach a summary of your qualifications and past experience as a firearms instructor.
  • Attach a copy of your course outline. All course outlines are required to meet the minimum content standards as established by the Bureau. A copy of the Bureau's training outline (Requires Adobe Reader. To download the FREE Adobe Reader, visit the Adobe® web site.) is sent with every application packet.

Processing the Firearms Instructor Application

  • The Bureau will conduct a background check on all applicants.
  • The Bureau will review instructor's course outline to ensure minimum training requirements are being met.

Instructor Responsibilities

  • The Bureau will provide an instructor number. The number must be used on all forms signed by the instructor.
  • Every concealed firearm permit applicant must receive instruction in the areas outlined by Section 53-5-704(8) and DPS administrative rule R724-4.

Falsification of any information in the application may result in denial or revocation of a permit and possible criminal prosecution.
