BCI is only authorized to distribute criminal history information from the state of Utah. If you need to obtain criminal history information from another state, contact that state directly.

The link http://www.doj.state.wi.us/dles/cib/sclist.asp provides a contact list for obtaining criminal history from all 50 states. BCI is providing this link as a service, but cannot guarantee the accuracy of the information contained in this link.

Every state has its own unique laws governing how it distributes criminal history information, as well as its own unique laws regarding expungement of criminal history information. BCI has no control over these laws or regulations.

For FBI background check requests, you must submit:

• A complete set of fingerprints on FBI form FD-258. The "Reason Fingerprinted" block must contain "Personal Records Check"
• A money order or certified check in the amount of $18.00, payable to the U.S. Treasury
• A letter stating that the check is needed for personal reasons (be sure to include your name and return address)

The findings of the background check will be mailed directly to the applicant. If no FBI record is found the fingerprint card will be stamped indicating "no record found". If there is a record found a copy of the record will be attached to the fingerprint card submitted (no stamp will be on the back of the fingerprint card).

The applicant cards and fingerprinting services are available at the Utah Bureau of Criminal Identification
(3888 W 5400 S, SLC) Monday thru Friday from 8:00 am to 5:00 pm at a cost of $13.00 per person. You can also check with local police departments, motor vehicle offices, or private industries for these services. Do not contact the FBI for fingerprinting.
