Click here to download the Homicide Report form in Excel format. Fill in appropriate boxes. (you'll notice that only the boxes colored yellow can be filled in.)
When you have entered all the necessary information, you have the choice of e-mailing or "snail-mailing" it to BCI.
To e-mail it, save the form, and then attach it to an e-mail addressed to macurtis@utah.gov
To "snail-mail" it, just print the completed form and mail it to:
