Government-issued picture identification that is valid (not expired) with the individual's name, date of birth, signature, or fingerprint is required to ensure proper identification, unless otherwise prohibited by law. The identification document must be legible and intact.

Exceptions: Exceptions to having an integrated signature or fingerprint include government-issued Military IDs, Passport Cards, Permanent Resident Alien Cards (Green Cards), and Employment Authorization Documents (EAD), as these do not contain visible signatures. Note that these exceptions are only permitted for in-person BCI services, including the Utah Criminal History Right of Access, expungement verification, Utah Criminal History record challenges, and fingerprinting services.

A combination (minimum of 2) of any of the following items may be used for acceptable verification. At least one of the documents must contain, at a minimum, an individual's photo and/or fingerprint, and a corresponding date of birth:

  • A punched (invalidated) driver's license
  • A punched (invalidated) identification card
  • Temporary identification issued by another state without a photo
  • Expired driver's license
  • Expired identification card
  • Birth certificate

  • Social Security card
